Responsibilities include: • Completing start-of-care and routine nursing visits • Assessing patient condition and developing care plans • Providing medication and disease-management education • Monitoring progress and preventing hospital readmissions • Coordinating care with physicians, therapists, and care team members • Identifying changes in condition and communicating appropriately • Documenting visits in the electronic medical record • Maintaining HIPAA and patient safety standards Training & Support You will not be sent out unprepared. We provide: • Structured onboarding • Ride-along visits with experienced clinicians • EMR training • Ongoing clinical mentorship and leadership support What We’re Looking For • Active, unrestricted RN license • 1+ year of nursing experience (hospital, SNF, rehab, clinic, etc.) • Willingness to work independently in the field • Reliable vehicle, valid driver’s license, and auto insurance • Ability to travel within the Lafayette coverage area • Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
