As a Registered Nurse (RN) at Enhabit, you will supervise, evaluate, and provide top-notch nursing care to clients in an array of senior living communities, including assisted living and independent living facilities. Our facility-based RNs enjoy: • Providing care to patients in the comfortable environment of their assisted living facilities • Personalized 1:1 patient care • The opportunity to follow each patient from admission to discharge You will collaborate closely with facility wellness staff to ensure clear communication and continuity of care for our residents and their families. In this role, you will also consult with physicians as necessary, provide updates on patient care, and act as the main contact for all parties involved in patient treatment. Key responsibilities include accurate OASIS collection, maintaining up-to-date medication profiles, timely reporting of lab values to physicians, attending weekly case conferences, and participating in monthly case manager meetings.
