St. Luke’s Health System in McCall, seeking a Home Health Case Manager to join our Home Health team. The Case Manager will occasionally be required to provide support to Hospice patients in the McCall area.
Position Overview As a Home Health Case Manager, you will play a key role in collaborating with physicians and multidisciplinary team members to create and manage individualized care plans for a designated caseload. This is a direct care role, involving travel within the McCall and surrounding regions to deliver in-home care from admission to discharge. Your work will ensure that each patient receives the highest standard of care, aligned with their needs and the care plan.

What You Can Expect in This Role
Full-time Schedule
:
Monday to Friday, 8:30 AM - 5:00 PM, with rotating evening on-call and weekend shifts, Opportunity to work primarily on weekends and cover occasional weekday shifts as well.
Patient Population
:
Home Health and Hospice patients within the McCall service area
Manageable Caseloads
:
Daily visit requirements are below the state and national average
Compensation
:
Paid hourly which includes drive time and mileage reimbursement for traveling within McCall and it's surrounding regions
Additional Pay
:
On-call pay and applicable differential pay

Qualifications
Education
: ASN or BSN in Nursing
Experience
: 2 years of RN experience;
Home Health or Acute Care experience preferred
Licenses/Certifications
:
Current RN license in the state of Idaho;
Basic Life Support (BLS) certification
Other Requirements
:
Valid driver’s license and proof of auto insurance
