As a Home Health RN Case Manager, here’s a big-picture view of what you’ll do:
• Complete accurate and timely OASIS Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments
• Develop, implement, and update individualized patient care plans
• Perform skilled nursing visits including assessments, medication management, wound care, and patient education
• Coordinate care with physicians, therapists, and interdisciplinary team members
• Ensure compliance with Medicare, state, and agency regulations
• Maintain thorough and timely clinical documentation in the EMR
• Provide education and support to patients and caregivers

A few must-haves for Home Health RN Case Managers:
• Graduate of an accredited nursing program and active RN license in Kansas
• CPR certification
• Knowledge of state and federal home health regulations
• Clinical proficiency in nursing specialty, strong communication skills and compassion
