A Brief Overview
Your responsibilities will include care planning, coordination of services, and education for interdisciplinary teams managing patient care effectively and efficiently, ensuring the best possible outcomes.
Your Responsibilities Include:
• Utilizing clinical expertise and problem-solving skills to assess patient needs and make recommendations to physicians, ensuring comprehensive care.
• Implementing the nursing process with proficient assessment skills during Start of Care visits, post-hospital evaluations, patient recertifications, discharges, and supervisory visits.
• Collaborating with physicians and the healthcare team to establish individualized care plans with the patient at the center.
• Using clinical protocols and assessing resource utilization to attain positive outcomes from admission to discharge.
• Managing changes in care plans, assessing significant changes in conditions, and conducting necessary evaluations.
• Educating patients and families on disease management, community resources, and self-care practices as directed by physicians.
Essential Qualifications

Required Education, License/Certification, or Work Experience:
• Registered Nurse (RN) - Board of Nursing Upon Hire
Preferred Experience:
• 1-3 years in home health or hospice care
