Registered Nurse (RN) – Homecare Extended Hours Triage

Full-Time

We are currently seeking a Full-Time Registered Nurse (RN) – Extended Hours Triage to support our homecare patients during evenings, nights, weekends, and holidays. This role is critical in ensuring timely clinical assessment, guidance, and coordination of care outside of standard business hours.
Position Summary

The Extended Hours Triage RN serves as a clinical resource for homecare patients, caregivers, and field clinicians during non-traditional hours. This role focuses on telephonic triage, clinical decision-making, symptom management, and coordination of urgent home visits or escalations as needed. The RN ensures continuity of care while promoting patient safety, comfort, and appropriate utilization of services.
Key Responsibilities
• Provide clinical triage and assessment via phone to homecare patients, caregivers, and on-call staff during extended hours.
• Utilize evidence-based protocols to assess symptoms, determine urgency, and provide clinical guidance.
• Coordinate urgent or PRN visits with field clinicians when necessary.
• Collaborate with physicians, supervisors, and interdisciplinary team members to support appropriate care decisions.
• Accurately document all assessments, interventions, and communications in the electronic medical record.
• Reinforce patient and caregiver education related to symptom management, medications, and when to seek further care.
• Identify changes in patient condition and escalate concerns appropriately to ensure patient safety.
• Support continuity of care by communicating relevant updates to day-time clinical teams.

Schedule
• Full-time position supporting evenings, nights, weekends, and holidays
• Specific schedule to be discussed based on operational needs

Qualifications
• Active New Jersey RN license in good standing
• Minimum of 2 years of clinical nursing experience (homecare, hospice, med-surg, or triage experience preferred)
• Strong assessment, critical-thinking, and clinical decision-making skills
• Excellent verbal communication and documentation skills
• Ability to work independently while collaborating with a remote interdisciplinary team
• Proficiency with EMR systems (Homecare/Homebase experience a plus)
• Valid driver’s license and reliable transportation

In compliance with New Jersey's Pay Transparency Act, the total compensation range for this position is USD $80,000 to USD $135,000. When determining an offer, VNA Health Group considers factors such as the scope and responsibilities of the role, the candidate's work experience, education/training, relevant skills, internal pay equity, and market and organizational conditions. The total compensation range reflects the total package, including base salary, overtime, bonuses, add-on incentives, special clinical compensation, retirement plans, and all other benefits.
