Location: Avera McKennan Hospital Worker Type: Regular Work Shift: Primarily days with rotating weekends and holidays (United States of America) Pay Range: The pay range for this position is $31.00 - $46.25, with actual pay dependent upon experience. Position Highlights Join our innovative Hospital at Home team, transforming healthcare delivery by providing hospital-level services to patients within the comfort of their own homes. As a core member of our care team, you will: • Redefine inpatient care by collaborating with hospitalists, therapy staff, pharmacy, and advanced practice providers (APPs). • Serve patients throughout the Sioux Falls region for 1-2 days, ensuring a seamless and personalized experience. • Participate in a high-satisfaction care model that patients love, often requesting our services upon returning to the hospital. Your Responsibilities As a Registered Nurse, you will be responsible for care planning, service utilization, care coordination, and educating interdisciplinary teams on patient management. You will: • Utilize your clinical expertise to assess patients' physical, psychosocial, emotional, spiritual, and bereavement needs. • Conduct Start of Care visits, post-hospital evaluations, and supervisory visits through proficient assessment skills. • Collaborate with physicians and the interdisciplinary team to develop individualized care plans. • Effectively manage resources and implement clinical protocols based on patient diagnoses. • Initiate adjustments to care plans as needed and determine if changes constitute a Significant Change in Condition. • Educate patients, families, and caregivers on disease processes and self-care strategies. Essential Qualifications • Must be a Registered Nurse (RN) licensed by the Board of Nursing upon hire. • Preferred: 1-3 years of experience in home health or hospice care.
