If you're driven to provide comfort, dignity, and support during life's most vulnerable moments, we would love to welcome you to our team.Key Responsibilities:As a Registered Nurse Case Manager, you will play a vital role in our interdisciplinary care team, ensuring that each patient receives personalized, high-quality care.Conduct patient admissions and ongoing nursing assessmentsDevelop and implement individualized plans of care based on each patient's needs and goalsCollaborate closely with hospice physicians and the interdisciplinary team (IDT)Supervise and coordinate care with home health aides, social workers, and chaplainsEducate patients and caregivers about disease progression, symptom management, and available resourcesEnsure timely and accurate documentation in compliance with hospice standardsPromote a culture of excellence, empathy, and accountability in every interactionQualifications:Current Registered Nurse (RN) License in Pennsylvania (required)Clinical nursing experience (hospice or home health preferred)Strong communication, organizational, and interpersonal skillsValid driver's license, reliable vehicle, and current auto insuranceAbility to travel within the assigned service areaComprehensive Benefits Package:At Vitality, we believe in taking care of our team the way we care for our patients.
