Home Instead Senior Care, located in Merrick, New York is seeking a Per Diem Registered Nurse (RN) to join our team.

PLEASE NOTE: TUESDAY & FRIDAY ARE THE MOST IMPORTANT DAYS TO BE AVAILABLE

Do YOU Qualify:
• 1-year Experience in Home Healthcare
• Ability to work independently
• Valid New York State RN License
• Display knowledge of and ability to follow NYS DOH regulations for Home Health Care.
• You are familiar with New York LHCSA procedures.
• Develop a personalized plan of care for all clients.
• Make periodic nursing assessments to review and revise the plan of care as necessary.

Registered Nurse Position Qualifications:
• At least 3 year’s experience in nursing
• At least 1-year Experience in Home Healthcare
• Ability to work independently
• Valid New York State RN or LPN License

Essential Job Functions / Responsibilities:
• Complete an initial home health assessment of client to determine home care needs.
• Conduct PCA Training Course and Conduct Orientations
• Conduct home visits as assigned, assessing the patient's condition, progress toward goals, providing patient care.
• Initiates the plan of care and makes necessary revisions as patient status and needs change.
