Registered Nurse (RN) VA Community Living Centers (CLC) are VA-operated nursing homes that provide long-term and short-term care, offering a homelike environment with skilled nursing, help with daily activities (ADLs), rehabilitation, and hospice for eligible veterans, focusing on restoring well-being, preventing decline, and providing comfort, often featuring resident-decorated rooms, family-friendly visits, and pet allowances to feel less institutional. Shift Availability : 0700 AM-0330 PM; 0730AM-4000PM; 0800AM-0430PM; 1030AM-0700PM; 1130AM-0800PM; 1230PM-900PM; 0330PM-1200AM. Scope of Care : Clinical Care : Administering medications, giving treatments (like nebulization, oxygen therapy), performing diagnostic tests, collecting samples, and wound management. Assessment & Planning : Conducting head-to-toe assessments, developing individualized care plans, and monitoring residents' health to detect changes. Coordination : Working with physicians, pharmacists, and other providers to coordinate care and implement orders. Communication : Serving as a liaison between residents, families, and the healthcare team, reporting health status changes. Documentation : Maintaining accurate patient records and managing paperwork for admissions and discharges. Emergency Response : Managing urgent medical situations and providing immediate care.
