Job Summary
As a Registered Nurse in our Nephrology department, you will provide exceptional clinical care in an acute inpatient setting, prioritizing patient and family-centered approaches. Your clinical competencies will be utilized through evidence-based practices across all phases of the nursing process, including assessment, planning, implementation, evaluation, and outcomes. You will actively engage in safety protocols, cost containment, and quality improvement strategies while adhering to the highest standards of compliance and regulatory requirements set by organizations such as TJC, SC DPH, and CMS.
This is a full-time position that may require weekend, holiday, and on-call scheduling.

Essential Functions

• Conduct thorough nursing assessments using appropriate area and age-specific standards, identifying subtle or significant changes in patient status and responses to interventions.
• Collaborate with patients, families, and the interdisciplinary healthcare team to develop individualized care plans that reflect assessed needs.
• Maintain and update care plans based on ongoing evaluations of the patient's condition.
• Set realistic, measurable goals and collaborate with patients and families to validate these goals.
• Respect and incorporate the cultural, spiritual, and belief systems of patients and families into care planning.
• Plan and coordinate a smooth discharge process to ensure continuity of care and necessary referrals.
• Provide direct patient care in accordance with established nursing plans and protocols.
• Delegate appropriate tasks to healthcare team members while ensuring timely completion and documenting assignments as necessary.
• Continually monitor the quality of care delivered and ensure comprehensive assessments of assigned patients.
• Organize and prioritize care based on patient and unit needs, utilizing resources efficiently.
• Prioritize patient safety by maintaining a safe environment and adhering to established patient care standards.
• Collect and utilize performance indicators to monitor the quality and effectiveness of nursing care.
• Engage in performance improvement monitoring and integrate findings into care plans as appropriate.
• Identify and assess patient and family learning needs, delivering education in an understandable manner, and evaluating its effectiveness.
• Communicate educational needs to other members of the healthcare team.
• Reflect on personal nursing practices regarding professional standards and seek opportunities for continued education and improvement.
• Document relevant information and provide oral communication that supports the care needs of patients and families.
• Act as the patient's advocate and coordinator of care by collaborating with healthcare providers and team members.
• Perform additional duties as assigned.
Supervisory/Management Responsibility
• This is a non-management position reporting to a supervisor, manager, director, or executive.
Minimum Requirements
• Education - Associate degree in Nursing required; Bachelor's degree in Nursing preferred.
• Experience - Minimum of one (1) year experience as a registered nurse.
In Lieu Of
• Successful completion of the Prisma Health Nurse Residency Program or a comparable residency from another organization may substitute for experience.
• Internal candidates with one (1) year of Licensed Practical Nurse (LPN) experience may qualify.
Required Certifications, Registrations, Licenses
• Must be licensed to practice as a Registered Nurse in South Carolina.
