Responsibilities:
The role of the clinical nurse is to deliver patient care through assessment, planning, implementation, and evaluation of patient needs with the highest professional standard while promoting nursing excellence through continued education and research, based on the Title 22 and Title 16, the ANA Scope & Standards of Practice, the Code of Ethics. Accountabilities1. SECTION 1: Initial Assessment and Nursing Diagnosis: RN will collect and analyze comprehensive data pertinent to the patient’s health or the situation to determine the diagnoses.2. Collects and prioritizes data in a systematic and ongoing process based on the patient’s immediate condition or anticipated needs.3. Completes a timely comprehensive patient assessment.4. Uses appropriate evidenced based assessment techniques in collecting and analyzing data.5. Involves the patient, family, significant other, and other health care providers in validating the data. Determine nursing diagnosis based on patient clinical data and information obtained in patient admission history.6. SECTION 2: Planning, Implementation and Patient Education: RN develops, implements, and coordinates a plan in a timely manner that prescribes strategies and alternatives to attain expected outcomes in a safe environment. Provides appropriate health education to patients and others as appropriate.7. Using the patient assessment and nursing diagnosis, develops and updates an individualized plan of care in collaboration with patient/family/significant other and other members of the medical team to meet the patients individualized needs, taking into consideration language preference and culture.8. Includes strategies within the plan that address each of the identified diagnoses or issues, as well as expected outcomes.9. Initiates and updates the patient plan of care, using the role based decision making process by considering the stability of the patient and incorporating data collection, patient assessment, implementation, evaluation, patient/family teaching and care coordination.10. Utilizing both dependent and independent RN functions, organizes both direct and indirect patient care efficiently and prioritizes appropriately to create a collaborative and communicative environment for all the members of the healthcare team.11. Clearly communicates the plan, as well as the associated goals of the plan, which include implementation pathways, timeline, methods to measure progress, and patient education.12. Initiates patient and family teaching plan at the time of admission and evaluates the effectiveness and documents their understanding on an ongoing basis.13. Uses health promotion and teaching methods appropriate to the situation and the patient’s developmental level, learning needs, readiness, ability to learn, language preference, and culture.14. SECTION 3: Evaluation, Quality of Practice and Patient Safety: RN evaluates progress toward attainment of outcomes which systematically enhances the quality and effectiveness of nursing practice and patient Safety.15. Evaluates the effectiveness of the patient’s plan of care in an ongoing, systematic and criterion based process including patient responses and the attainment of expected outcomes.16. Uses the results of quality improvement and evidence based research to initiate changes in nursing practice.17. Maintains consistent approach to “hand off” communications, including the use of SBAR, asking and responding to questions, ensuring the communication is accurate and pertinent to patient care, and reflects the stability of the patient.18. Participates in unit and hospital based quality improvement activities.19. Demonstrates an ongoing evaluation of the patient condition and environment and initiates strategies to optimize patient safety and reduce risk.20. SECTION 4: Education and Professional Practice Evaluation: RN attains knowledge and competencies that reflect current nursing practice and evaluates own nursing practice in -relation to professional practice standards and guidelines.21. Participates in ongoing hospital and unit specific educational activities.22.
