Job Summary
Under the general direction of nursing leadership and with provider collaboration and guidance, the Registered Nurse utilizes evidence-based practices in accordance with the State Nurse Practice Act and policies and procedures of Hartford Healthcare Hospital Outpatient Departments (HODs) and Medical Group, in a variety of clinical settings. This position facilitates care for a varied population of patients, which includes nursing standards of practice to meet the physical, emotional, spiritual, cultural, and educational needs of the patient and family.
Provides nursing within a non-acute setting in collaboration with inter-disciplinary teams and provides according to physical, emotional, spiritual, cultural, and educational needs of the patient and family.
Coordinates and provides holistic patient centered care.
Utilizes evidence-based practice in accordance with the department and / or medical group policies and procedures to ensure safe quality care and customer service.
Incorporates the four components of the Professional Practice Model of Nursing into their daily practice : Excel, Empower, Advocate and Engage.
Demonstrates and actively supports research utilization through inquiring about evidence-based practices.
Evaluates and reviews the plan of care working with the interdisciplinary team to meet the diverse needs of the patient and family.
Identifies and reliably documents problems and expected outcomes based on a plan of care for a varied population of patients.
Maintains confidentiality of all patient information.
Support two clinical therapeutic groups daily.
Facilitate activity group daily.
Qualifications
Bachelor’s degree in Nursing from an accredited School of Nursing.
Successful completion of nurse residency, if applicable.
One year of experience with a minimum of 1000 hours of clinical experience as a licensed RN, 5 years preferred.
Registered Nurse, State of CT.
Certified BLS Provider credentialed from the American Heart Association (AHA).
Additional certifications in specific specialties are preferred and may be required.
Must be able to effectively communicate information in English.
