Job Description
Position Overview
We are seeking a PRN Registered Nurse to provide admissions support and ongoing supervisory oversight for clients receiving non medical Personal Assistance Services under Ohio PAS, private pay, and veterans programs.
This is not a bedside nursing role.
This is not skilled home health.
The RN functions in an oversight, supervision, and quality role to ensure client safety, service appropriateness, caregiver competency, and regulatory compliance. The RN does not provide routine hands on care or skilled nursing services.
This role is ideal for an experienced RN who wants flexible work, defined scope, and a low stress environment without on call or hospital demands.
Compensation
Per visit pay structure
Rates discussed during interview based on visit type and scope
Mileage reimbursement provided
No unpaid on call expectations
Core Responsibilities include Client Admissions and Reassessments
• Participate in the admissions process to determine appropriateness of PAS services
• Review client information and functional needs
• Conduct reassessments when there is a reported change in condition, safety concern, incident, or service modification
• Focus assessments on functional status, safety, and service appropriateness
• No medical diagnosis or treatment is performed
Plan of Care Oversight
• Review and approve PAS Personal Care Attendant Plans of Care
• Ensure services are within PAS scope and aligned with assessed needs
• Update plans of care when services change, incidents occur, or safety concerns are identified
Caregiver Supervision
• Provide supervisory oversight of caregivers delivering PAS services
• Supervision may include in home visits, chart review, consultation, and documentation review
• Ensure caregivers understand assigned tasks, follow the Plan of Care, and operate within PAS scope
• Supervision is risk based and adjusted according to client acuity and caregiver experience
Training and Competency Oversight
• Verify caregiver competency using agency approved checklists and documentation
• Participate in orientation and in service education as needed
• Competency verification is documented and maintained in caregiver records
Incident Review and Follow Up
• Review client and caregiver incident reports
• Determine whether follow up actions are required such as additional supervision or Plan of Care updates
• Ensure appropriate escalation to medical providers when needed
• The RN does not provide medical treatment
Consultation and Availability
• Serve as a clinical and safety resource to caregivers and agency staff
• Availability is defined by agency policy and scheduled expectations
• This role does not include on call hospital style coverage
Quality Assurance
• Participate in quality assurance and performance improvement activities
• Assist with trend review, incident analysis, and corrective action planning related to client care and caregiver performance
Scope Limitations
This role is limited to non medical Personal Assistance Services.
The RN does not:
• Administer medications
• Perform wound care
• Provide skilled nursing services
• Deliver medical treatments
Documentation
• Document assessments, supervision, Plan of Care reviews, incident follow up, and competency oversight using agency approved tools.
Qualifications
• Active Registered Nurse license in the State of Ohio in good standing
• Experience in home care, community health, long term care, or supervisory nursing preferred
• Strong assessment, documentation, and communication skills
• Understanding of non medical home care scope and caregiver supervision
• Reliable transportation and willingness to travel locally
• CPR certification required
