Be the difference.Job SummaryResponsible for the development and maintenance of the quality of care delivered to stroke patients and their movement throughout the continuum of care. Serves as a clinical expert serving as a role model and resource to patient and family, nursing staff, other clinical staff, physicians and the community. Establishes and fosters collaborative working relationships with physicians involved in the clinical care of stroke patients.Essential Functions All team members are expected to be knowledgeable and compliant with Prisma Health's purpose: Inspire health. Be the difference.Develops, evaluates and maintains a stroke program that meets and exceeds the recommendations of the American Stroke Association, the Brain Attack Coalition and the JCAHO Disease Specific Standards. Responds, when possible, to Stroke Alert calls from the other areas of the hospitals.Serves as a clinical expert and resource to patient care services personnel and other health care professionals within the hospital, assisting with care for complex and highly specialized patient care situations throughout the continuum and in the community.Promotes and develops personal and professional growth through the development of self and patient care services staff. Participates in identifying needs, planning and implementing educational programs within specialty areas for hospital health care providers, patients, families and/or community groups. Improves the care of stroke patients through facilitation and coordination of prevention programs and community education.Facilitates and monitors data from the stroke registry. Is involved in process improvement and research activities related to the care of stroke patients.Facilitates and coordinates the Stroke Team meetings, coordinating their efforts in evaluation of data and process improvement.Assumes responsibility for the ongoing program management for stroke, across the continuum from EMS and hospital entry to return to home.Performs other duties as assigned.Supervisory/Management ResponsibilitiesThis is a non-management job that will report to a supervisor, manager, director or executive.Minimum RequirementsEducation - Bachelor's degree in Nursing. Master's degree in Nursing preferred.Experience - Three (3) years Nursing experience. Experience in Neuro nursing preferred.
