$5,000 SIGN-ON BONUS FOR FULL-TIMEHomecare Case Manager RN - M-F 8a-430p, every 4th weekend and 2 holiday rotationSummary of Position: The Primary Nurse is responsible for direct patient care.Responsibilities:Responsibilities of Position:Provide competent nursing care to assigned patient through skilled symptom management, emotional support, appropriate teaching, and timely response to patient and family needs, including;Routine Home Care Visits.End of Life/Continuous Care.Referrals and Admissions.Care Center Assistance.Chart Reviews.Other duties as assigned.Coordinate IDG planning for assigned patients.Participate in the IDG analysis of the patient, resulting in the development, implementation, and revision of the IDG Plan of Care.Teach, supervise, and evaluate the care provided by volunteers, hospice aides, and primary caregivers.Facilitate continuity of care by coordinating admission of home-care patients to inpatient settings (hospital, nursing home), as needed.Explain hospice services and benefits in regards to Medicare, Medicaid, and Private insurance, and know when to use Palliative Care Interventions and ABNs.Obtain and enter orders from Medical Directors and Physicians when deemed appropriate.Order appropriate Durable Medical Equipment (DME) for the patient.Perform competent initial nursing assessments.Work evenings and weekends as needed to meet the needs of the patients and families, and the agency's backup schedule.Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements.Maintain clinical competency for assigned responsibilities and procedures.Other duties as assigned.Qualifications:Education and Experience:Graduation from an approved School of Nursing.Current RN licensure in the State of Ohio.Minimum of two years of experience in Medical/Surgical, Oncology, or community health nursing.Hospice experience preferred.Special Skills and Requirements:Shall have a valid driver’s license.Excellent customer service skills.Detail-oriented with excellent communication, documentation, and organizational skills.Ability to follow instructions.Ability to work independently without supervision.Travel to other sites as needed. Travel may be in inclement weather.Ability to lift 25 lbs of supplies or equipment.Requires various amounts of sitting, standing, twisting, turning, bending, lifting, and walking.Ability to assist with patient care activities such as ambulation, transfers, and positioning.About CompanyEverHeart Hospice was established in 1980 and is a legacy non-profit, community-based hospice organization.
