The RN will ensure quality care, monitor outcomes, and work closely with caregivers, families, and healthcare professionals. Key Responsibilities: • Conduct comprehensive in-home assessments and reassessments • Create and revise individualized care plans based on clinical findings • Provide medication administration and skilled nursing interventions • Monitor and document changes in client health and status • Communicate regularly with clients, caregivers, and care teams • Initiate referrals to appropriate healthcare or community resources • Maintain timely, accurate documentation in accordance with regulations • Participate and facilitate caregiver in-service programs.
