Job Description Registered Nurse (RN) - Wound Care & Ventilator Specialist in Wisconsin Person-Directed Living - Premium Pay - County Benefits We are seeking a dedicated and compassionate Registered Nurse (RN) with strong wound care and ventilator experience who wants to work in a collaborative, team-based environment where Person Directed Living is the top priority. As an integral member of the household clinical team, you will support both the medical and social needs of each villager, helping them live with dignity, choice, and the highest possible quality of life. Position overview Reporting to the Nursing Supervisor, the RN functions as a key clinical leader within the household, planning and supporting person-directed living according to physician orders, nursing orders, care plans, and villager choice. In this role, you will provide advanced nursing care with a special focus on complex wound management and ventilator-dependent villagers, ensuring safe, evidence-based, and compassionate care. Compensation and benefits New base pay hiring range: 38.21-44.30 USD per hour, based on experience (before shift differentials). PM shift: additional 3.00 USD per hour (2:00 pm-10:30 pm). NOC shift: additional 2.00 USD per hour (10:00 pm-6:30 am). All weekend hours: additional 2.00 USD per hour, regardless of shift. Overtime paid for all hours worked over 8 hours in a single day. Amazing County benefits for any 50% posting or higher, with eligibility for the Wisconsin Retirement Pension at 60% (24 hours/week) or more. Key responsibilities Provide person-directed nursing care by planning, administering, and evaluating villager care based on care plans, physician orders, nursing orders, and villager likings. Deliver specialized wound care, including comprehensive assessment, staging, dressing selection, infection monitoring, and ongoing evaluation in collaboration with the interdisciplinary team. Manage ventilator-dependent and respiratory-compromised villagers, including monitoring ventilator settings, performing respiratory assessments, coordinating with respiratory therapy, and responding to changes in status. Assess villager medical and social needs, organize documentation, and assist providers during acute care rounds, ensuring timely and accurate communication. Report and document changes in condition promptly and accurately according to policy, escalating concerns as appropriate. Report infections to the Infection Control/Skin Care Coordinator, implement appropriate precautions, and support best practices in infection prevention and skin integrity. Perform supervisory functions as delegated by the Neighborhood Manager and Registered Nurse Specialist, including supervising, directing, and evaluating Nursing Assistants. Educate nursing and household team members on policies, procedures, wound and skin care protocols, ventilator and respiratory care guidelines, and person-directed living principles. Attend nurse meetings, neighborhood/household meetings (household councils, community circles, huddles), and care conferences, actively contributing to care planning and problem solving. Ensure all households participate in Quality Improvement efforts; perform professional audits and observations to confirm staff competency and support quality outcomes, including wound and ventilator care metrics. Promote team empowerment by providing leadership, vision, coaching, and counseling, and by modeling positive, resident-focused attitudes and behaviors. Maintain a safe, comfortable, and functional household environment, monitoring for hazards and adhering to best practices in sanitation, infection control, fire safety, disaster preparedness, and emergency care; investigate and report incidents and accidents and observe for safe work habits. Qualifications Degree in Nursing. Current State of Wisconsin RN license. Current adult BLS/CPR certification (documentation required prior to start date and must be maintained). Successful completion of a Post Offer Examination (POPE). Prior experience in wound care (e.g., complex wounds, pressure injuries, post-surgical wounds) and caring for ventilator-dependent or high-acuity respiratory villagers/residents preferred. Skills and competencies Proficiency with common medical and nursing equipment, including devices used in wound care and ventilator/respiratory management. Strong ability to assign, supervise, and review the work of others while fostering a supportive, team-based culture. Ability to read, comprehend, and interpret medical reports, charts, xrays, lab reports, policy and procedure manuals, and medical documentation. Ability to prepare accurate and timely documentation, including lab sheets, chart reviews, incident reports, care plans, and wound/ventilator care records. Confident use and interpretation of medical terminology in both written and verbal communication. Excellent communication skills with physicians, team members, pharmacy, villagers, and families/legal representatives. Ability to ensure compliance with federal, state, and local regulations, as well as accepted professional standards and organizational policies. Commitment to person-directed living as a core component of villager health, wellbeing, and satisfaction. Demonstrated ability to model positive attitudes, resilience, and collaborative behavior in a fast-paced care environment. Physical requirements Ability to stand and walk for extended periods. Ability to lift and carry up to 50 lbs occasionally and up to 25 lbs frequently. Ability to push/pull up to 50 lbs of force. Ability to stoop, kneel, bend repeatedly, and reach high, low, and forward. Repetitive finger movements and the use of both hands. Clear verbal communication and adequate vision to safely and effectively perform all RN duties.
