Your focus will be on delivering top-notch nursing care, especially for those with complex wound issues and ventilator dependencies, ensuring the best possible quality of life. Key Responsibilities: • Deliver personalized nursing care by implementing and evaluating resident care plans with consistent attention to physician orders and resident preferences. • Provide specialized wound care, conducting thorough assessments, selecting appropriate dressings, monitoring infections, and collaborating with the healthcare team. • Oversee the care of ventilator-dependent residents, including monitoring settings and coordinating respiratory therapy while responding to any changes in their condition. • Conduct assessments of medical and social needs, maintain accurate documentation, and assist healthcare providers during rounds to support effective care delivery. • Communicate changes in condition promptly, document findings accurately, and escalate concerns as needed. • Report infections, implement necessary precautions, and champion best practices in infection prevention and skin health. • Supervise and guide Nursing Assistants to ensure optimal patient care within the household. • Educate team members about policies, procedures, and care standards for wound and ventilator management. • Participate actively in care conferences, neighborhood meetings, and quality improvement initiatives while promoting a culture of teamwork. • Maintain a safe and clean environment while adhering to sanitation, infection control, and emergency procedures. Qualifications: • Degree in Nursing with an active Wisconsin RN license. • Current adult BLS/CPR certification. • Experience in wound care and care for high-acuity residents preferred.
