Wound Care Nurse
Full-Time with On-Call rotation
What You'll Do:
As a Wound Care Nurse, you’ll assess, treat, and monitor wound conditions in collaboration with a skilled interdisciplinary team. Your care planning and interventions will enhance wound healing and improve our residents' comfort and quality of life. You will serve as a practitioner, consultant, educator, and facilitator to elevate the standard of care for individuals with complex wound care needs.
Key Responsibilities
• Provide expertise and oversight for the wound care and prevention program for residents and staff.
• Conduct thorough wound assessments and create individualized care plans for patients with skin complications.
• Perform direct wound care interventions, including dressing changes, debridement, and wound irrigation.
• Review and implement wound care & prevention policies and procedures.
• Monitor the progress of wound healing and evaluate treatment effectiveness.
• Educate residents, families, and staff on wound care protocols and prevention strategies.
• Work closely with physicians, wound care specialists, and other healthcare professionals to coordinate care.
• Maintain accurate documentation of wound assessments, interventions, and outcomes for regulatory compliance and reimbursement.

What You'll Bring:
Qualifications include:
• Valid state RN nursing license.
• Degree in nursing, epidemiology, biology, or microbiology preferred.
• 2+ years in wound care management, preferably in Long-Term Care.
• Knowledge of research-backed wound care and cleansing products.
• Certified Wound Care Nurse (CWCN) Certification preferred.
• Strong clinical assessment, organizational, and critical thinking skills.
• Compassion and empathy for residents with complex wound care needs.
• Interest in the nursing needs of the aged and chronically ill.
• Excellent communication and interpersonal skills.
• Able to work effectively in a multidisciplinary team environment.
