Farmsteads of New England is a unique Farm designed to support individuals diagnosed with Intellectual/Developmental Disabilities, as well as individuals diagnosed with Acquired Brain Disorders. We provide support, care, and lifelong memories.
What will I do?
As a Nurse Trainer, you will coordinate the care for your assigned individuals, teach others how to implement quality care, provide medications and conduct regular quality assurance activities, in short… help our individuals live their best lives. You will ensure that our medical services are well-coordinated, effective, and supportive for individuals, families and team members.
Daily Responsibilities
• Prioritize kindness, dignity, respect, communication, and integrity.

• Provided and maintain a safe working environment.
• Provide leadership while executing the mission, vision and goals of Farmsteads of New England.
• Ensures all program staff follow all agency and program philosophy, ISA goals, objectives, and coordination responsibilities, to include ensuring that all medical documentation is completed in a timely fashion.
• Other duties as assigned.
• Provide education and certification for direct support staff in medication administration per He-M 1201
• Serve as the primary healthcare coordinator for assigned individuals
• Collaborate with primary care, specialty, and mental health providers to ensure continuity of care
• Conduct initial and ongoing staff training related to medication administration
• Perform quality assurance audits to ensure compliance with He-M 1201
• Coordinate with medical providers and pharmacies to resolve medication-related issues
• Provide on-call support for staff requiring medical guidance or triage on a rotating basis.
• Conduct in-person or telephonic assessments to determine necessary medical action
• Prepare for and participate in audits, licensing visits, and state certification reviews
• Maintain ongoing adherence to all applicable state healthcare regulations
• Act as a resource for initial diagnostic evaluations and the development of care plans
• Provide expertise regarding medical and psychiatric diagnoses and current treatment protocols
• Attend annual Individual Service Agreement (ISA) meetings and quarterly or monthly team meetings as appropriate
• Participate in monthly Developmental Disabilities Nurses of New Hampshire (DDNNH) meetings
Position Requirements:

• Current RN license in New Hampshire
• Strong clinical judgment, documentation, and communication skills
• Ability to work independently and collaboratively as part of a care team
• Ability to communicate effectively verbally and in writing and ability to use good judgment.
• Must have basic computer knowledge.
• Strongly prefer a candidate that will have a demonstrated understanding of and competence in serving individuals with I/DD and Acquired Brain Disorders.
• A valid driver’s license.
• Ability to pass all applicable DCYF, BEAS, Local Law, Federal and other required background screenings per applicable regulations.
• 18 Years or older at the time of hire
• U.S. Citizen or possession of necessary papers allowing holder to work legally in the United States.
• Personal/Professional experience caring for others is a significant plus.
Compensation
• Competitive Pay ($70,000 to $85,000 based on experience)

• Compensation is dependent on Education, Experience (professional/personal) and/or certifications (if any).
Schedule
• Primarily Monday through Friday. One to two weekend days, once a quarter and based on the need to provide training/quality checks with weekend-only staff.
• Eight-hour shifts between the hours of 6am and 6pm. With typical schedules between 8a/9a to 4p/5p.
