The RN VAD Coordinator oversees the development, implementation and maintenance of all aspects of care for patients with a ventricular assist device (VAD) in collaboration with the Heart Failure team. To coordinate and manage the patient care of this high-risk population throughout the continuum to optimize the patient's clinical, functional and psychosocial status. Provides the communication link for coordination of care among the multidisciplinary health care team for VAD patients. Demonstrate respect, dignity, kindness and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background. Additional duties include: • Manages the care of the high-risk ventricular assist device (VAD) patient population across the continuum (acute care inpatient, outpatient, clinic and extended care facilities) in collaboration with the Heart Failure Specialists, CT Surgeons and other members of the multidisciplinary team. This involves advanced assessment, planning and coordination of care, modification of the treatment plan as indicated and monitoring and evaluation of outcomes. • Provides clinical consultation and education related to the management and care of VAD devices for staff, physicians as well as other disciplines who may have contact or care responsibilities for this population. • In collaboration with the Heart Failure team, oversees performance improvement and certification activities associated with VAD patients to improve patient outcomes. • Develops a therapeutic relationship with the VAD patient and family and maximizes the patient's participation and control in their own recovery. • Facilitates the access and utilization of community resources and organizations to assist in the patient's recovery. • Demonstrates leadership skills such as time management, coordinating and meeting multiple patient and family needs and requests, meeting physician and team member expectations, setting priorities and networking. • Demonstrates commitment to professional growth and competence. • Performs other duties as assigned. Education, Credentials, Licenses: Registered Nurse in state where work is being performed Bachelor's degree in Nursing ACLS Certified BLS Certified Specialized Knowledge: Strong interpersonal, communication and education skills Extensive knowledge of nursing standards and practice related to heart failure management Kind and Length of Experience: Minimum of 4 years nursing experience (at least two years of critical care or VAD experience) FLSA Status: Exempt Right Career.
