Job Description
As a Registered Nurse, you will play a crucial role in delivering high-quality nursing care to patients in their homes. Your work will include coordinating care with healthcare professionals and ensuring continuity of care that adheres to physician directives, legal regulations, and agency policies.
Key Responsibilities:
• Deliver exceptional clinical services while adhering to infection control standards.
• Collaborate with the care team from patient admission to discharge.
• Execute clinical nursing assessments as required by federal and state standards.
• Confirm patient eligibility and medical necessity for services according to payer and agency policies.
• Develop and adjust personalized care plans in collaboration with community healthcare providers.
• Ensure that care plans meet patient needs and facilitate revisions with physician approval.

Required Qualifications:
• Active and unrestricted RN licensure in your state.
• Valid driver's license and vehicle insurance, with reliable transportation.
• Current CPR certification.
• Ability to provide care in diverse home environments regardless of the demographics of clients.
Preferred Qualifications:
• Over 1 year of RN experience.
• Able to work independently with strong communication and organizational skills.
Compensation is competitive and based on factors such as local market conditions and individual qualifications. The hourly pay for this role ranges from $28.27 to $50.48, based on full-time employment.
