
Registered Nurse - Weekend Shift in Campbell Station, AR
Job Description
As a Registered Nurse in Home Health, you will take charge of providing and directing nursing care to patients in their homes, as outlined by physicians and in line with applicable laws, regulations, and agency policies. You will also coordinate comprehensive care plans with other healthcare professionals, ensuring continuity and quality of care through effective communication and collaboration.
Key Responsibilities:
• Clinical Competence:
• Initiate, develop, implement, and revise care plans in collaboration with the physician and other health professionals.
• Oversee care provided by home health aides and LPNs, providing necessary training and assigning tasks according to regulations.
• Conduct required supervisory visits.
• Documentation and Care Delivery:
• Deliver high-quality clinical services in alignment with best practices and infection control standards.
• Perform comprehensive assessments (OASIS), including medication reconciliation, accurately and in a timely manner.
• Document patient visits per policy and sync information in a timely fashion according to LHC policy.
• Quality:
• Undertake initial and comprehensive nursing evaluation visits to ascertain eligibility and medical necessity.
• Ensure timely and effective communication with appropriate staff regarding patient care and service issues.
• Maintain open communication with physicians, patients, and families to guarantee the highest standards of care.
• Teamwork:
• Follow direction from the Clinical Director and Executive Director while timely completing assigned tasks.
• Assist in onboarding new staff and act as a preceptor for trainees.
• Participate in performance improvement initiatives and efforts to minimize unnecessary patient hospitalizations.
• Engage in on-call and weekend rotations as needed to address patient requirements.
• Contribute to agency utilization management activities.

Required Qualifications:
• Current and unrestricted RN licensure in the state of practice
• 1+ years of Home Health experience
• Valid Driver's License, vehicle insurance, and access to a reliable vehicle or public transportation
• Current CPR Certification
• Ability to work in diverse home settings, regardless of clients' backgrounds
Preferred Qualifications:
• Proven ability to work independently
• Effective communication, writing, and organizational skills
Compensation is based on various factors such as local labor markets, education, experience, and certifications. The hourly pay for this role ranges from $28.27 to $50.48 for full-time engagement.
