General Responsibilities:
We provide culturally competent care to patients from birth through late adulthood, utilizing a multidisciplinary team approach. As a registered nurse you are an essential member of the team delivering care in areas such as family medicine, primary care, pediatrics, obstetrical and gynecological care, medication assisted treatment, trans and behavioral health. Responsibilities include, but are not limited to, competent patient assessment, promptly prioritizing triage calls, determining appropriate patient disposition, managing nurse visits, providing direct patient care, and assisting providers as needed. The registered nurse acts as a resource person for medical assistants and other team members.

Required Qualifications:
Current State of Rhode Island Registered Nurse license
Ability to communicate effectively and work collaboratively with patients, providers, other members of the healthcare team, other disciplines, and community agencies
Professionalism, self-starter, leadership skills, triage skills, documentation skills, and teaching skills
CPR certification required
Preferred Qualifications:
Three or more years nursing experience preferred, new RN graduates will be considered
Ambulatory Health Care experience preferred

Significant Job Functions:
Possess basic nursing critical thinking skills (i.e.,assessment, triage, knowledge of medical terminology)
Perform proactive chart review in anticipation of provider and patient needs and huddle with provider/MA as needed
Ensure the safe delivery of care to patients (i.e.,newborn, infant, child, adolescent, adult, geriatric)
Evaluate and assess patient’s healthcare needs and determine patient disposition (i.e.,provider appointment, urgent care, ER, selfcare)
Triage phone calls and evaluate patient healthcare needs, referencing up-to-date telephonic triage protocols
Perform nurse visits and provide evidence-based health education and information at an appropriate levelof understanding for the patient
Administer vaccines and other injections as ordered by a provider and in accordance with agency procedures and policies
Perform treatments ordered by providers such as dressing changes, respiratory treatments, etc.
Support various population needs, including trans health and mental health as part of primary care
Provide family planning and HIV counseling and documentation in accordance with Title X and based on the needs of our patient population
Accurately obtain and document vital signs (i.e.,weight, blood pressure, BMI, pain assessment, temperature)
Review externally ordered labs and diagnostic imaging and notify provider of abnormal results according to company policies and workflows
Support obstetric patients in pre-natal and post-natal care, triage, and education via nursing and/or provider visits, and telephonically (Applicable to Woonsocket and WW only)
NST Certification and experience helpful but not required
Lactation Counselor Certification helpful but not required
After appropriate orientation/trainingof OB and pediatric specialties, possess ability and flexibility to rotate through dependent on staffing needs. (Applicable only to Woonsocket)
Support pediatric patients through accurateassessment and triage and determine appropriate patientdisposition. Administer immunizations and providerordered treatments.
Prior pediatric emergency or primary care experience helpful but not required
Experience in pediatric catheterization helpful but not required
Perform medication reconciliation as needed and process prescriptions per established workflows
Assess patients for potential communicable diseases and utilize infection control precautions. (i.e.,isolation of patient with TB or Chicken Pox)
Act as a resource for other staff
Notify patients of test results under the guidance and knowledge of providers, offer education, and assess patient understanding
Receive and verify verbal orders from providers, assuring accuracy of the order
Develop and implement nursing care plans based on patient evaluation
Maintain line of communication with supervisor, providers, and other staff members
Actively participatesin performance improvement activities
Respond to codes and rapid responses
Utilize standing orders and SOP’s
Maintain OSHA and HIPPA guidelines
Adhere to organizational policies and procedures
Utilize electronic medical record alerts and act on them when appropriate
Maintain standard precautions
Assume responsibility for professional continuing education
Perform other tests, duties and special projects as assigned
Utilize training resources including RN manual, workflows, and instructional materials to perform at top of license
Physical Effort/ Environment:
This role is onsite
Moderate physical activity, walking, standing, stooping, and occasional lifting.
Potential exposure to blood-borne pathogens, biohazardous materials, and infectious diseases (i.e.,tuberculosis, COVID-19).
Manual dexterity required for injections, immunizations, and computer use.
Work Schedule Demands:
As scheduled during hours of operation (inclusive of weekend and evening hours as assigned) with occasional unscheduled overtime

Have sensitivity to the patient population we serve, by identifying cultural, social, and economic characteristics. Applies verbal and non-verbal communication skills in patient interactions and utilizes teach-back, active listening, written materials, and visual aids.
Able to document clearly, competently, and concisely in the medical record. Ability to maintain confidentiality of information and communicate in a professional manner with patients, families, other staff, and outside contacts
