Your Role:
As a Registered Nurse specializing in Wound Care, you will undertake the assessment, treatment, and ongoing monitoring of wound conditions in coordination with our interdisciplinary team. Your care planning and interventions will significantly enhance the wound healing process and improve the quality of life for our residents. You will serve as a practitioner, resource, consultant, educator, and facilitator, driving high-quality care for individuals with complex wound care needs.
Key Responsibilities:
• Oversee and provide expertise in the wound care and prevention program for residents and staff.
• Conduct comprehensive wound assessments, developing tailored care plans for patients with skin complications.
• Deliver direct wound care interventions, including dressing changes, debridement, and wound irrigation.
• Review, update, and ensure compliance with wound care and prevention policies and procedures.
• Monitor wound healing progress and assess the effectiveness of various treatment modalities.
• Educate residents, families, and staff on wound care protocols and preventive measures.
• Collaborate closely with physicians, wound care specialists, and other health professionals to coordinate optimal care.
• Maintain accurate documentation of wound assessments, interventions, and outcomes to adhere to regulatory standards and facilitate reimbursement.

Qualifications:
• Valid state RN nursing license.
• Degree in nursing, epidemiology, biology, or microbiology is preferred.
• Minimum of 2+ years in wound care management, ideally in a Long-Term Care setting.
• Familiarity with evidence-based wound care and cleansing products.
• Certification as a Wound Care Nurse (CWCN) is preferred.
• Strong clinical assessment, organizational, and critical thinking skills.
• Genuine compassion and empathy in caring for residents with complex wound care needs.
• Interest in addressing the nursing needs of the elderly and chronically ill.
• Excellent communication and interpersonal abilities.
• Ability to effectively collaborate in a multidisciplinary team environment.
