Job Description
Position Summary: Join our dedicated healthcare team as a Wound, Ostomy, and Continence Care Advisor. In this crucial role, you will assess, plan, implement, and evaluate personalized care for patients facing challenges related to wounds, ostomies, and continence. You will act as a consultant, delivering direct patient care and education while also fostering staff development in your areas of expertise. Additionally, you will have a significant role in our innovative wound telemedicine program.
Essential Duties & Responsibilities:
• Craft and update personalized care plans for each patient.
• Thoroughly assess the physiological, psychosocial, environmental, and spiritual needs of each patient.
• Utilize strong physical assessment skills to evaluate patient conditions.
• Administer medications safely and efficiently.
• Apply critical thinking skills to effectively delegate and assign tasks.
• Collaborate with coordinators and licensed personnel to enhance care planning.
• Maintain a safe environment for patients, visitors, and staff by adhering to safety standards.
• Communicate clearly and effectively with patients, families, and healthcare team members.
• Exemplify leadership by delegating and organizing tasks while educating colleagues.
• Understand the relationship between cost, quality, and efficiency in patient care.
• Provide expert consultation on the management of patients with:
• Ostomies or continent diversions.
• Altered skin integrity.
• Percutaneous tubes.
• Urinary and fecal incontinence.
• Assist colleagues in keeping current with knowledge in wound, ostomy, and incontinence care.
• Conduct product evaluations and provide thoughtful recommendations based on findings.
• Offer enteral consultation for patients and staff across various regions.
• Document patient records in adherence to agency policies, ensuring timely completion of clinical notes and care conferences.
• Operate medical equipment effectively to enhance patient care.
• Remain informed of nursing and home care agency policies and procedures.
• Engage in the Performance Improvement program and offer constructive feedback to management.
• Uphold professionalism and utilize appropriate phone etiquette.
• Create a welcoming atmosphere for all visitors while maintaining client confidentiality.
• Participate in accreditation programs and assist with backup calls as needed.
• Ensure all clinical documentation is submitted promptly according to agency guidelines.
• Perform additional duties as assigned to enhance team efficiency.
Education/Experience:
• At least two years of nursing experience within the last four years.
• Strong communication and assessment skills.
• Bachelor's degree in Nursing from an accredited program is a must.
Licenses, Certifications and/or Registrations:
• Valid RN license.
• Graduated from an accredited WOC Nursing program and possess Board Certification.
• Current CPR certification.
• Valid Driver's License and auto insurance.
• Completed annual education in HIV/AIDS and Domestic Violence.
Working Conditions:
