Registered Nurse/Licensed Practical Nurse in Bloomington, IL

Position Summary The RN/LPN is responsible for providing in-home skilled care to a child in a home and school setting. This role ensures confidentiality, safety, and quality of care are maintained. Proper and timely documentation of care in an electronic health record (EHR) is essential in this position. Credentials and Experience • Unencumbered Illinois Registered Professional Nurse (RN) license or Licensed Practical Nurse (LPN) license required • Valid Illinois driver's license required • Experience in home health preferred • Experience with pediatric clients preferred Duties/Responsibilities • Conducts client assessment at appropriate times • Documents all findings in client’s electronic health record in a timely manner • Administers and documents medication administration appropriately • Completes skills following care plan • Adheres to infection control procedures • Maintains client safety • Maintains client’s comfort following care plan • Communicates (verbal, written, nonverbal, listening) effectively with client, family, agency, and other caregivers • Responds to changing situations and expectations • Takes on challenges and learns new skills • Works independently • Manages time appropriately • Follows family’s home guidelines • Adheres to HIPAA and maintains confidentiality • Address problems or questions to the appropriate person at the appropriate time Physical and Mental Demands • Sit, stand, bend/stoop, and reach above shoulder level • Ambulate to perform duties • Lifts and carries up to 50 lbs. and exert up to 100 lbs. force to push/pull • Performs fine and gross motor skills • Reads, reaches, and changes settings on equipment • Has visual ability to assess a patient and accurately prepare and administer medication • Responds to verbal communication from clients, families, and other healthcare givers including the ability to respond to emergency signals • Detects heartbeats and pulse when taking a blood pressure • Functions effectively under stress Work Environment: • Provides own transportation to and from clients’ homes • Works in partnership with clients’ families in their homes • Follows family guidelines pertaining to use of electronic devices, use of internet, parking, dress code, and/or areas as outlined We offer competitive compensation and benefits packages.
