Provide skilled nursing care in a rehabilitation and long-term care setting, playing a critical role in promoting resident health and well-being. Responsibilities: • Communicate promptly with physicians, families, and interdisciplinary teams regarding significant changes in residents’ conditions • Complete resident assessments and medication administration with adherence to safety protocols • Perform wound care and assessments as required • Support CNAs by responding to call lights and monitoring resident status • Lead floor operations by assessing and directing situations to ensure quality care • Report any suspected abuse, neglect, or misappropriation to appropriate leadership promptly • Maintain accurate and thorough resident records with detailed charting • Carry out additional nursing duties as assigned Qualifications: • Wisconsin Registered Nurse license in good standing required • Current CPR/BLS certification mandatory • Minimum two-year nursing degree required; bachelor’s degree preferred • At least one year of relevant nursing experience preferred • Demonstrated accuracy, attention to detail, and dedication to high-quality care • Ability to work effectively within multidisciplinary teams Location: Tomah, WI, offers a welcoming community atmosphere with access to natural beauty including parks and recreational areas.
