
Rehab/Long-Term Care Nurse (RN) | Sevierville, Tennessee in Pigeon Forge, TN
Job Description
Key Responsibilities:
• Conduct comprehensive nursing assessments within 8 hours of patient admission.
• Develop and implement individualized care plans based on assessment findings.
• Collaborate with patients, families, and interdisciplinary teams to identify teaching needs and clinical goals.
• Document all patient assessments, interventions, and responses accurately and timely according to policy.
• Participate actively in rehabilitation programs, prioritizing patient education and encouragement.
• Initiate discharge planning from admission, preparing patients and families for transition.
• Communicate pertinent patient status changes effectively among healthcare providers to maintain continuity and safety.
• Provide leadership by directing and supervising ancillary nursing personnel, delegating tasks appropriately.
• Ensure adherence to safety protocols, promoting a safe environment for patients and staff.
• Assume charge nurse responsibilities as requested following orientation.
• Engage in ongoing professional development and contribute positively to team dynamics.
