The Care Manager plays a crucial role in implementing our Value Based Care Management program by delivering exceptional telephonic case management to CareFirst members. Collaborate closely with members, caregivers, providers, and our interdisciplinary care team to craft effective care plans that enhance member experiences and improve health outcomes. As a Care Manager, you'll advocate for members and apply motivational interviewing techniques, guiding them toward achieving their health goals while providing support throughout their care journey. Start Date: February 23, 2026 Schedule: Monday - Friday, 8am - 5pm ET Training: The first six weeks from 9am-5:30 EST. Full attendance during this time is crucial to your onboarding, with time-off requests reviewed on a case-by-case basis. Salary: $85,000 annually, along with a comprehensive benefits package. Essential Job Functions: • Engage with members, caregivers, and providers to develop comprehensive care plans and strategic interventions that address needs throughout the care continuum. • Collaborate with PCPs, specialists, and other clinicians to meet healthcare goals and implement effective care plans. • Continuously assess member care needs and progress, making necessary adjustments to address changes in health status or care preferences. • Coordinate patient education according to care standards and relevant health concerns, tailored to individual member needs. • Identify community resources and social determinants of health, facilitating referrals based on member needs. • Assist members in coordinating additional tests, imaging, and specialty consultations. • Conduct medication reconciliation at the start of the care plan, after health status changes, and monthly to monitor for efficacy and interactions. • Facilitate smooth transitions of care, ensuring appropriate follow-up and oversight of the care plan post-discharge. • Maintain thorough documentation to reflect member progress and obstacles to achieving care plan objectives. • Adhere strictly to Value Based Care Management program guidelines. • Meet productivity and quality metrics as required. • Complete all mandatory training and annual competency evaluations. • Actively participate in team huddles and contribute to collective clinical knowledge. • Stay current on clinical knowledge through self-directed learning. Specific Skills/Attributes: • Strong case management and motivational interviewing skills. • Self-directed, highly organized, and adept at multitasking with excellent problem-solving capabilities. • Ability to meet deadlines consistently. • Exceptional oral and written communication skills. • Ability to provide exceptional customer service and effectively communicate with all stakeholders. • Experience collaborating with various administrative and professional staff. • Resilience and adaptability in a fast-paced environment. • Proven skills in engaging members with outstanding customer service. • Ability to compile concise medical records that accurately narrate the member's healthcare story. • Proficiency in data analysis and documentation for reporting needs. • Proactive in incorporating quality improvement processes into practice. • Experience in medically oriented care plan documentation. • Adept at working within a matrix organizational structure. Qualifications: • Current multi-state compact Registered Nurse licensure in residence state, with the ability to obtain additional licenses. BSN preferred. • Preferred training in motivational interviewing. • 3-5 years of diverse clinical experience, with a strong preference for telephonic case management experience. • Proficient in electronic medical records, word processing, spreadsheets, and presentation software. • Maximize technology use, including Microsoft Teams, Word, Excel, and Outlook. • This position is home-based; candidates must ensure compliance with HIPAA requirements and meet minimum internet connectivity standards. • Effective communication skills with members, team members, physicians, and plan representatives. • Ability to read and interpret scientific literature, presenting information effectively to diverse audiences.
