Remote RN Care Manager in Salt Lake City, UT

Job Description We are looking for a dedicated Registered Nurse (RN) with a valid license to join our team as a Care Manager. In this role, you will work with a Medicaid/Medicare population in South Carolina, conducting assessments, developing care plans, providing education, and connecting members to vital community resources. Strong computer skills and attention to detail are essential as you multitask between systems, communicate with members and providers over the phone, and maintain accurate documentation. Key Responsibilities • Conduct comprehensive assessments of members within regulated timelines to identify those who qualify for care management based on clinical judgment and health changes. • Develop and implement personalized care coordination plans in collaboration with members, caregivers, healthcare professionals, and support networks. • Monitor care plans to evaluate effectiveness, document interventions, and propose necessary adjustments. • Maintain an ongoing caseload of members for regular outreach and proactive management. • Promote the integration of services across behavioral health, long-term services and supports (LTSS), and community resources. • Facilitate interdisciplinary care team meetings to ensure coordinated member care. • Utilize motivational interviewing techniques and Molina clinical guideposts to support members in effecting positive change. • Assess barriers to care and coordinate resources to address member concerns. • May provide consultations, resources, and recommendations to peers as required. • Potentially handle complex member cases and medication management, including medication reconciliation. Work Schedule This is a remote position, working Monday through Friday from 8:00 AM to 5:00 PM EST, with flexibility to work 1 to 2 nights a week until 6 PM EST. No weekends or holidays required. Qualifications • At least 2 years of experience in healthcare, particularly in care management or a medical/behavioral health setting. • Current and unrestricted Registered Nurse (RN) license. • Valid driver’s license, reliable transportation, and car insurance for travel as needed. • Familiarity with electronic medical records (EMR) and HIPAA regulations. • Proficient in Microsoft Office and online databases.
