Join Our Team as a Remote RN Care Manager! We are seeking a passionate and skilled Registered Nurse (RN) to become an essential member of our care management team. This remote position allows you to make a meaningful impact on the Medicaid/Medicare population by conducting assessments, creating tailored care plans, providing valuable education, and connecting members to vital community resources. Key Responsibilities: • Conduct thorough assessments of members to determine eligibility for care management services. • Collaborate closely with members, caregivers, and healthcare professionals to design and execute effective care coordination plans. • Maintain consistent outreach to manage an ongoing caseload of members. • Facilitate the integration of services across behavioral health and long-term support systems for comprehensive care. • Identify barriers to care and offer tailored care coordination support to overcome these challenges. • Utilize motivational interviewing techniques to empower and educate members about their health. • Lead interdisciplinary care team meetings to foster collaboration and holistic care approaches. Requirements: • Active and unrestricted RN license in South Carolina or compact license. • Minimum of two years of healthcare experience, preferably in care management or behavioral health. • Outstanding communication and interpersonal skills. • Proficient computer skills and a keen attention to detail. • Ability to work Monday to Friday, from 8:00 AM to 5:00 PM EST, with some flexibility for occasional evening shifts.
