Responsibilities:
• Guide staff in clinical and case management decisions, emphasizing excellence in end-of-life care and symptom management.
• Ensure the development of personnel qualifications and implementation of agency protocols.
• Facilitate staff professional development through home visits, evaluating clinical performance and creating development plans.
• Ensure compliance with regulatory policies, focusing on cost-effective clinical outcomes.
• Accountable for achieving desired clinical and operational performance metrics.
• Utilize clinical software to track team performance, identify educational opportunities, and oversee patient care progress.
• Collaborate with internal and external parties to resolve patient care issues efficiently.
• Oversee daily patient assignments based on staff competencies and client needs.
• Coordinate timely referrals and service delivery, ensuring assessments of patient needs are ongoing.
• Develop and update personalized care plans for all patients, ensuring effective communication with all involved physicians.
• Assess staffing needs and coordinate hiring processes to meet service demands in your area.
• Recruit, train, coach, and evaluate staff, managing personnel evaluations within prescribed timelines.
• Analyze team and patient metrics in line with agency initiatives to drive improvements.
• Actively participate in preceptor programs and educate students regarding hospice and palliative care issues.
• Be proactive in addressing customer needs, offering quick and pleasant responses.

Qualifications:

Nursing:
• Must be a licensed registered nurse in the State of Connecticut.
• Preferred: Bachelor’s Degree in Nursing; Diploma in Nursing required.
• Experience in hospice care and familiarity with OASIS and case management certifications are preferred.
• Basic keyboarding skills and the ability to manage electronic communications are essential.
