Are you a compassionate, motivated Admissions & Resource RN who loves being on the road and thrives in a dynamic environment? AdvisaCare is seeking an energetic, organized, and efficient Registered Nurse with hospice experience to join our Hospice Family based in Gilbert, AZ, serving the North Central Valley and surrounding areas.
This role is perfect for a nurse who values the mission of hospice care and enjoys driving to meet patients where they are. As an Admissions & Resource Nurse, you will play a vital role in welcoming patients and families into hospice services, completing admissions, providing clinical support across teams, and offering emotional guidance during a critical time. Your goal will be to help patients live their final days with dignity, comfort, and compassion.
Responsibilities include completing hospice admissions, providing resource and clinical support to field staff, conducting supervisory and clinical observation visits, and collaborating with the interdisciplinary team. You will actively participate in team meetings, offering input on the development, review, and ongoing revision of individualized patient and family plans of care.

Requirements
• Current Arizona License in good standing
• Empathetic approach to patients and caregivers
• Participate in field visits daily, weekly office meetings as indicated
• Ability to adapt to patient's status and needs on a full time on call basis
Job Responsibilities:
• Initial and on-going assessment of patient's physical, functional, psychosocial and environmental needs
• Implement the individualized plan of care and revisions as needed
• Compliance with AdvisaCare Hospice Policies and Procedures, Medicare compliance
• Consulting with and educating the patient/family on disease process, self-care techniques, end-of-life care, dealing with issues of ethical concern
• Supervision of paraprofessionals providing services to patient
• Preparing clinical and progress notes, participating in IDG conferences
• Participate in Hospice performance improvement program
• Maintain high level of customer satisfaction with our clients, families and caregivers
• Communication of information using current process and technology within organization
