This includes ensuring that an appropriate restorative plan of care is established and developing a program to meet the restorative needs and rehabilitation goals of the resident; • Functions as a resources person for nursing staff, and ensures continuity of residents rehab/restorative regimen on the nursing units • Ensures continuity of Residents’ rehab / restorative regimen on the nursing units • Collaborates with therapists and other members of interdisciplinary team to assume rehab / restorative responsibilities as an adjunct to skilled therapy, or when formal skilled therapy transitions to restorative care. • Administrative / Staff Development • Participates in (responsible for) interviewing, hiring, and on-boarding process of qualified prospective employees; • Collaborate with Nurse Educator in support of staff education, training, and professional development. (includes needs assessment for annual competency training and that the safety needs of the residents are being met). • Responsible for ensuring adherence to nursing standards of care and policies and procedures relative to residents’ rehabilitation / restorative needs. Includes managing corrective action process for staff in order to improve overall performance; • Responsible for ensuring adherence to and compliance with all rules and regulations specified by various governing and regulatory bodies (IDPH, CARF, etc.); • Responsible for ensuring timely completion of staff annual performance appraisals. Includes development and oversight of associated performance improvement plans when appropriate; • Department Management • Facilitates and supports department based and organizational improvement plans and continual quality improvement plans; Tracks all restorative programs for all residents in health care and submits periodic statistical reports to Director of Nursing (including QA reports); • In collaboration with Healthcare Administrator and Director of Nursing, development of departmental goals and benchmarks and monitors ongoing progress toward accomplishing established goals. Includes implementing change processes needed to meet goals. • Facilitates departmental meetings to ensure open flow of communication to and from staff. Ensures that Stand Up! Is part of the daily communication expectations. • Works with staff in establishing and maintaining mobility and all ADL programs. • Regularly participates in residents’ care conferences and ensures restorative and safety care plan interventions are implemented. • Monitors and ensures staff documentation of Restorative programs and ADLs is appropriately completed and entered in computer program for each resident. • Oversees all ambulation programs to ensure that all ambulation needs of residents are met and reviews for appropriate documentation. • Participates in weekly nursing leadership meetings, weekly • Rehab/Medicare meetings, monthly QA meetings, and others as required or assigned. • Responsible to learn and use standard functions within software and know basic operation of computer to enhance efficiency in resident/patient service, assessment and/or clinical documentation. Responsible to use computers and software as required by work environment in order to enhance communication, performance and knowledge. • Ensures awareness of and compliance with Presbyterian Living Corporate Compliance policies including but not limited to confidentiality and security of information, and concern for the well being of residents and patients. Demonstrates behaviors and completes work in a manner consistent with these policies. Education: Registered Nurse licensed in the State of Illinois. BSN or equivalent preferred. Qualifications • Active, RN License in the State of Illinois. • Restorative/Rehab Nursing Certification. • CPR Certification Required - Accredited by American Heart Association. • 3 years experience as a staff RN and geriatric patient experience. • Genuine interest in working with older adults. Knowledge of aging changes and common health problems of geriatric patients. • Previous nursing management experience preferred.
