Shifts Available: Per Diem, Full Time Evening
FT: Monday - Friday 1:00pm To 9 Pm

Location: The Osborn, Rye, NY
Position Summary
The Osborn is seeking a compassionate and detail-oriented Admission Registered Nurse (RN) to join our team on a per diem basis. This role is essential in overseeing new resident admissions, managing intake, and ensuring a smooth transition into our community. The Admission RN will conduct clinical assessments, provide direct nursing care, and coordinate with interdisciplinary teams to deliver exceptional resident-centered care.
We are looking for a professional RN who demonstrates strong clinical judgment, organizational skills, and a commitment to excellence in long-term care.
Key Responsibilities
• Coordinate and oversee all aspects of new resident admissions and intake.
• Conduct thorough nursing assessments and initiate individualized care plans.
• Provide direct nursing care to residents as needed, ensuring safety and comfort.
• Supervise and delegate tasks to Nursing Assistants in compliance with facility standards.
• Accurately document resident assessments, care plans, and progress notes.
• Administer medications and treatments as prescribed by healthcare providers.
• Monitor residents’ conditions and promptly report changes to appropriate staff.
• Ensure compliance with infection control and safety protocols.
• Collaborate with interdisciplinary team members to support residents’ health and well-being.
• Promote professionalism and uphold the mission, vision, and values of The Osborn.
Qualifications
• Active NY State RN license and current CPR certification required.
• Prior experience in admissions, long-term care, or skilled nursing preferred.
• Strong clinical assessment, documentation, and organizational skills.
• Excellent communication and interpersonal abilities.
• Ability to manage multiple priorities and work effectively as part of a team.
• Knowledge of regulatory requirements for long-term care facilities.
