
RN Care Coordination Lead (Non-Medical Intake & Quality) in Bountiful, UT
Job Description
The Mission
As our RN Care Coordination Lead, you serve as the clinical authority for our agency. You are the primary decision-maker for admissions, conducting expert evaluations to ensure every client we admit is a safe and appropriate fit for non-medical personal care. In this role, you act as our Lead Care Designer, partnering directly with the Director to develop high-quality, personalized service plans that empower our Veterans and seniors to thrive independently at home.
Role Overview
This is a non-clinical, administrative-clinical role. Your expertise will be utilized for:
• Comprehensive Assessment: Evaluating physical, social, and environmental needs.
• Quality Care Planning: Designing roadmaps for daily support.
• Case Management: Navigating the lifecycle of client care.
• Note: This position does not require medication management, wound care, or direct clinical nursing procedures.
Why Intake Matters in This Role
The "Intake" phase is the blueprint for our service delivery. As our Intake Specialist, you will:
• Determine Eligibility: Conduct mandatory initial in-home assessments to verify that client needs align with non-medical care standards.
• Capture Clinical History: Meticulously organize medical histories and personal preferences to set a professional tone for the client’s journey.
• Design ADL Support: Define specific Activities of Daily Living (ADLs) required and facilitate strategic caregiver matchmaking based on client personality and care needs.
Key Responsibilities
• Admission Assessments: Lead clinical interviews with potential clients and families to determine service appropriateness based on agency policy.
• Quality Care Plans: Collaborate with the Director and Office Team to design "care plans that are individualized and outcome-focused.
• VA Case Management: Manage the care lifecycle for Veteran clients, ensuring plans align strictly with VA authorizations and Community Care Network (CCN) standards.
Qualifications
• Active Utah RN License: Required.
• Assessment Expertise: Strong critical thinking skills to identify physical and psychosocial needs during intake.
• Collaborative Mindset: A drive to work with non-clinical staff and leadership to maintain elite care standards.
• Empathy & Communication: Exceptional ability to build immediate trust with seniors, Veterans, and their families.
Job Types: Part-time, PRN
Pay: $38.50 - $41.00 per hour
Expected hours: 5 – 20 per week
