Role Overview We are seeking a Care Manager to join our team in Houghton County, MI. In this role, you will coordinate patient care across multiple services, ensuring a seamless experience and improved health outcomes . You’ll work closely with providers, patients, and community resources to support chronic disease management, preventive care, and overall wellness. Key Responsibilities • Develop and implement individualized care plans. • Coordinate referrals and follow-up care across medical, behavioral health, and social services. • Provide patient education and support for self-management. • Monitor progress and adjust care plans as needed. • Collaborate with interdisciplinary teams across various organizations to improve patient outcomes. Qualifications • Michigan Registered Nurse (RN) licensure. • Experience in primary care, care coordination, or case management. • Strong communication and organizational skills. • Ability to work collaboratively in a team-based environment. • Commitment to serving underserved populations and advancing health equity.
