A-Line Staffing is hiring RN Care Managers in Kent, New Castle, and Sussex County, DE! This is a Full-Time / 40 hours per week Contract to Hire position.
Location: Delaware (New Castle County, Kent County, Sussex County)
Schedule: Monday–Friday, 8:00 AM – 5:00 PM EST
Pay Rate: $35–$40/hour (based on experience) Plus mileage reimbursement!
Contract Length: 6-month contract to hire
Overtime: No OT expectations
Position Overview:
A-Line Staffing is looking for LTSS Care Managers to support LTSS members through comprehensive care coordination, in-home assessments, and service planning. This role focuses on helping members remain compliant with level-of-care requirements while addressing immediate needs and coordinating community and healthcare resources.
This is a field-based role requiring regular home visits throughout Delaware.
Key Responsibilities
• Conduct in-home and site visits to assess member needs and overall health status
• Assist with immediate member needs and support LTSS team efforts related to level-of-care redeterminations
• Develop, assess, and maintain individualized long-term care service plans
• Coordinate care between members, families/caregivers, providers, specialists, and community resources
• Monitor member progress, outcomes, and service plan effectiveness, making recommendations as needed
• Educate members and caregivers on available services, benefits, care procedures, and provider instructions
• Provide resource support related to housing, employment, independent living, justice, foster care, and participant direction
• Maintain accurate documentation to ensure compliance with state, federal, and payer regulations
• Collaborate with healthcare providers and partners to ensure continuity of care
• Provide feedback to leadership to improve quality of care and service delivery
• Perform additional duties as assigned while adhering to all policies and standards
Required Qualifications
• Bachelor’s degree is required
• 2–4 years of related Case Management experience
• Active Delaware RN License or Compact RN License
• Valid driver’s license in good standing
• Dedicated home workspace for documentation and administrative tasks
• Candidates must reside within the state of Delaware in one of the counties listed
• Reliable transportation required for member home visits
Preferred Qualifications
• Prior experience as a Service Care Manager or in LTSS roles with a health plan
• Performance Expectations:
• Complete approximately 4–5 member assessments per week
• Complete 4–5 redetermination assessments daily, as assigned
