Schedule
Sunday – Saturday, 10 shifts variable 7:00 AM – 5:30 PM & weekend rotation with call.
Job Description
The Nurse Case Manager in the Emergency Department (ED) is responsible for establishing, coordinating, and maintaining processes to enhance patient throughput to the most appropriate level of care while facilitating interdisciplinary care across the continuum for the ED. This role requires collaboration with patients/families, multidisciplinary teams, physicians, community resources, and payers to ensure efficient and effective patient progression and care management.
Responsibilities
• Assess patient status to determine appropriate level of care using InterQual Criteria.
• Collaborate with medical teams to identify patients suitable for inpatient admission, observation, or outpatient status.
• Coordinate discharge planning with referral to appropriate care levels.
• Manage care for patients with specific needs, including those with mental health issues.
• Ensure efficient and cost-effective management of care processes.
• Work with other case managers, social workers, and leadership teams to enhance patient care.
Required Experience / Certifications / Licensure
• Associate’s degree in Nursing.
• Current, unrestricted professional nursing license in any U.S. state, Puerto Rico, or a U.S. territory.
• Basic Life Support (BLS) certification by the American Heart Association is required throughout employment.
• Case Manager Certification (CCM or ACM) must be obtained within 2 years from hire if not already held.
• A minimum of 2 years of current care/case management experience in the Emergency Department.
• At least 2 years of patient care experience in a hospital, home health, or skilled nursing setting.
• Proficient with Electronic Medical Records, Outlook, Microsoft Word, and Excel.
• One year of travel experience preferred.
