Case Manager RN – Acute Care
Location

Assignment Duration
This is a 13-week contract position.
Schedule
Day shift, 5 days a week, with varying start times between 07:00 AM and 08:30 AM. Every other weekend is required.
Job Description
The Case Manager RN will be responsible for overseeing discharge planning and care coordination for patients within an acute care setting. The role requires effective collaboration with an interdisciplinary team to assess needs, develop care plans, and facilitate transitions of care for diverse patient populations, including neonates, adolescents, adults, and geriatrics.
Responsibilities
• Conduct needs assessments and develop individualized care plans.
• Coordinate patient care services across various departments.
• Ensure compliance with CMS and HIPAA guidelines.
• Utilize Epic for charting and documentation of patient care activities.
• Manage discharge planning and patient education to ensure smooth transitions.
Required Experience / Certifications / Licensure
• Active Registered Nurse (RN) license in Alaska.
• Basic Life Support (BLS) certification.
• 2 years of acute care experience, with prior case management experience preferred.
• Certified Case Manager (ACM) certification is a plus.
• Experience using Epic EMR is required.
