Job Description
JOB TITLE: RN Case Manager
• This PRN role requires a mix of case management, recertifications, starts of care (SOC), resumptions of care (ROC), with occasional new hire orientations and supervisory reviews, among other RNCM assessments and services.
• On occasion, Skilled Nursing visits may be requested but are optional.
• At least 1 year of Case Management experience and home health agency (or equivalent) experience are required.
• Must have experience with OASIS and NON-OASIS assessments.
• 1 or more years of experience in home health care, wound care, lab draws, and OASIS assessments is a plus.
• This role can be built to fit your schedule and preferences - whether that's:
• as a 2nd or 3rd job (working around your primary schedule)
• as a consistent PRN / Part-Time-As-Needed position to keep busy but not too busy
• building up to a permanent Part-Time or potentially Full-Time position
• or becoming a road warrior if a Full-Time schedule is desired (requires regular travel 2-3 hours from home)
SERVICE AREA: SouthWest Ohio
• Primary service area is focused in Dayton + Dayton Metro Area, however coverage across a 90-minute region of surrounding communities is required (including Middletown, Franklin, Troy, Piqua, Springfield, parts of West / Southwest Columbus Metro, and nearby areas).
• Typical travel distances require a 30-60 minute daily radius (or less), with travel between 1-2 hours (max.) required on occasion.
JOB LOCATION OPTIONS:
• Qualified applicants may reside in or near any of the service areas so long as travel requirements are fulfilled to all stated communities.
SCHEDULE TYPE:
• Flexible and customizable. We'll build a schedule as close to home as possible - that fits your availability and preferences.
• Typical hours are between Monday - Friday (weekdays) during the day (8am - 5pm).
EMPLOYMENT TYPE: PRN with potential to increase to consistent Part-time if desired (not required)
• No On-Call requirements / No Overnight shifts required.
• HCN will expand this role into Part-Time or even Full-Time for the right candidate.
• Weekend, Overtime, Respite & PRN services are offered when available but are not required.
• PRN / Part-Time RNCMs receive hourly compensation (paid weekly, on Fridays).

To support our growth and the increasing volume of client care throughout Ohio, we are seeking a highly skilled, experienced, accountable, and self-motivated home health PRN RN Case Manager (RNCM) to join our agency and serve existing and returning clients throughout Dayton + Dayton Metro Area and surrounding SouthWest Ohio communities.
• This is a PRN / As Needed position, with hours and shifts varying week to week, including some weeks having low or no hours at all. Candidates seeking long-term employment are preferred, however short-term or temporary employment will be considered.
• This position will be built to fit your schedule (with potential to expand into a permanent Part-Time or Full-time position if desired).
• Typical travel requires a 30-60 minute daily radius (or less), with travel between 1-2 hours (max.) required on occasion. Additional hours are available if willing to travel up to or beyond 2+ hours as needed (optional, not required).
For those pursuing career growth: a defined path for advancement to Assistant Program Administrator, Program Administrator, Clinical Administrator, Business Administrator, QA / Compliance leadership, and other positions will be supported.

Compensation:
• PRN Hourly Wage: $35 - $40/hour
Job Summary
As a PRN RN Case Manager, you will be instrumental in coordinating and providing patient care and ensuring optimal outcomes.
• With core skills in managed care, patient assessment, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
• Your premium skills in critical care and infectious disease care will be valuable in handling complex cases, while your relevant skills in home and community care and supervising experience will enhance your ability to lead and support a multidisciplinary team.
• You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.
Reports to: Regional or Clinical Administrator / Director of Nursing
Responsibilities/Requirements of an RN Case Manager include:
• RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
• RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
• RN to Document/Communicate/Coordinate patient status/change in condition to care team.
• RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
• Willing to travel surrounding counties.
• Minimum of one (1) years of nursing supervisory experience. Home care experience preferred.
Qualifications
• Graduate of an accredited School of Nursing.
• Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
• CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
• 1 year Experience in case management and home health required, with 2+ years of home health experience highly preferred.
• Experience in a supervisory role, wound care, lab draws, and OASIS assessments is a plus.
• Willingness to travel throughout all stated counties in this service area to care for clients is required.
• Strong background in nursing, patient care, and confidentiality.
• Strong attendance and dependability track record with previous employers.
• Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
• Familiarity with HIPAA regulations and medical terminology.
• Current Driver's License and reliable transportation.
