Dependable Nursing is a home healthcare agency providing skilled nursing and therapy services to pediatric, dependent adult, and senior patients in the home setting. We are seeking an experienced Home Health RN Case Manager who is interested in growing into a Director of Nursing (DON) role within the next year.
This position will begin as an hourly RN Case Manager to support patient care as our census grows. As responsibilities expand, this role will transition into structured leadership training under our current DON, with a clear pathway into the DON position.
Position Summary
The RN Case Manager provides direct patient care and oversees the coordination of services for a caseload of home health patients. In addition to case management duties, this role includes gradual exposure to clinical leadership responsibilities in preparation for advancement into a Director of Nursing role.
This is an ideal opportunity for a home health nurse who is ready to grow beyond field visits and develop skills in clinical oversight, compliance, and team leadership.
Schedule
• Hourly, non-exempt position to start
• Caseload and hours will grow with agency census
• Structured leadership development plan to transition into salaried Director of Nursing role within approximately 12 months

Primary Responsibilities (RN Case Manager)
• Manage and coordinate care for an assigned home health caseload
• Complete Start of Care, Recertification, Resumption of Care, and Discharge OASIS assessments
• Develop and update patient care plans in collaboration with physicians and interdisciplinary team
• Provide skilled nursing visits as needed
• Communicate changes in patient condition to physicians and care team
• Ensure documentation is accurate, timely, and compliant with Medicare and payer regulations
• Participate in case conferences and care coordination
Leadership Development Responsibilities (DON Training)
• Under the mentorship of the Director of Nursing
• Clinical support and guidance for field clinicians
• Chart audits and documentation review
• Participation in QAPI and quality improvement initiatives
• Survey readiness and regulatory compliance activities
• Assistance with onboarding and clinical orientation of new staff
• Incident review and clinical follow-up
• Collaboration with Administrator and DON on clinical operations
Requirements
• Active California RN license
• Minimum 2 years of home health experience
• BLS/ CPR
• Experience caring for pediatric, dependent adult, and geriatric patients in the home setting
• Strong experience with OASIS assessments and home health documentation
• Proficiency with G-tubes, tracheostomies, medication management, & wound care
• Experience providing clinical oversight and delegation to LVNs
• Solid understanding of home health regulations, care planning, and physician order management
• Excellent time management, communication, and organizational skills
• Desire to grow into a Director of Nursing role
Preferred
• Experience with chart review, QA, or compliance activities
• Experience training and/or supporting other clinicians
• Wound Care Certified
• IV Therapy Experience
• ACLS
• BA in Nursing
• Base Access clearance (or ability to obtain)
