Shift and Status Full time, 8:00 a.m. to 4:30 p.m., variable days and on call weekends Hybrid: Home and Facility Visits, travel to patients' homes, nursing facilities, and hospitals to provide direct care in Niles/Buchanan and surrounding areas. We provide both inpatient and outpatient medical services throughout Southwest Michigan. Scope of Work Under the direct supervision of the Nursing Manager, the Hospice Nurse Case Manager is responsible for coordinating and delivering compassionate end-of-life care to patients and their families. This role involves assessing patient needs, developing individualized care plans, and collaborating with an interdisciplinary team to ensure the highest quality of hospice care.
