What Youll Do as a Registered Nurse Case Manager
• As a Registered Nurse Case Manager, youll be the cornerstone of our care teamhelping patients live their best quality of life. Each day youll:
• Coordinate and manage individualized hospice plans of care
• Deliver hands-on nursing care and emotional support to patients and families
• Educate caregivers and guide them through the hospice journey
• Collaborate with physicians, social workers, and chaplains to ensure holistic care
• Respond promptly to changes in patient needs with expertise and compassion
What to Expect
This full-time RN Case Manager role is 40 hours per week, MondayFriday, with a rotating on-call schedule for evenings and weekends. Youll work independently while always supported by a caring team.
What Youll Bring
• Current Illinois Registered Nurse (RN) license
• Graduate of an accredited nursing program (BSN preferred)
• At least 1 year of medical/surgical, oncology, or hospice nursing experience (2+ years preferred)
• CHPN certification preferred (or obtained within 2 years)
• CPR certification (within 3 months of hire)
• Strong clinical judgment and ability to work independently
• Local residence with ability to respond to on-call within 30 minutes

• $78,000$108,000 annually, based on experience and education
