RN Case Manager Palliative Care
Location: Enfield Region

Position Purpose:
The Palliative Care RN is responsible for patients and families in the palliative care program. Provides Palliative care clinical consultation.
Position Details:
Schedule: Monday - Friday, 8 am - 5 pm, one weekend every 4 weeks and two rotating holidays per year. NO ON CALL REQUIREMENTS!
Coverage Region: Enfield
Bonus: $10,000 sign-on bonus paid out over two years.
This position is 100% home care providing services in in patients private homes.
What you will do:
• Collaborates with manager in the development of performance measurements for the Palliative Care Program to ensure continuous quality improvement; ensures implementation
• Motivates/supports employees for exemplary performance on internal and external customer service, productivity, and accuracy
• Provides statistical/budgetary data to appropriate Palliative care program.
• Works collaboratively with all departments to ensure execution of quality delivery of services
• Ensures that stakeholders are satisfied with services provided; improvement plan developed when necessary.
• Develops tracking outcomes of enrolled patients and ensures information is available for review
• A commitment to quality palliative care provision and transition to hospice when appropriate
• Exercises sound clinical judgment in oversight of Palliative Care Program
• Knows, understands, incorporates, and demonstrates the Trinity Health Mission, Vision, and Values in behaviors,
• practices, and decisions.
• Minimum Qualifications:
• Graduate of an approved nursing education program.
• Current Registered Nurse licensure in the state of practice.
• CPR certification
• Hospice/Palliative Care certification preferred.
• Home care experience, strong clinical knowledge base, Hospice experience
• Computer skills, strong interpersonal and organizational skills.
• Palliative Care experience, demonstrate ability to use problem solving, critical thinking and priority setting skills.
• Must be able to work independently and on a team,
• Ability to perform diversified duties with time limitations,
• Ability to develop, coordinate and evaluate programs in Palliative care and end-of-life care.
• Valid driver’s license and reliable transportation required
