Job Description
We are seeking a dedicated and compassionate Registered Nurse Case Manager to join our growing healthcare team. The primary focus of the RN Case Manager is to assess, plan, implement, and evaluate patient care to ensure optimal outcomes. This position collaborates with healthcare providers, patients, families, and other team members to develop and monitor care plans, provide patient education, and promote quality, cost-effective healthcare services.
Responsibilities
• Obtain all outside records as needed: Specialist, Hospitals, Urgent Care, Pharmacy Immunizations, etc.
• Communicate with referral specialists to assist patients with education and flu reminders.
• Set follow up reminders for providers review 30 days prior to patients next appointment.
• Assist in GPRA compliance by noting goals and identifying patients due for screenings or immunizations, and review providers schedule the week prior.
• Assist in identifying patient resources, example: food pantries, housing needs, and transportation services. Refer to BH Case Manager.
• Check provider mailbox, review before placing in providers mailbox.
• Sort through providers mail, identify requests that require F2F OV, assist in scheduling if needed.
• Review medications, enter lab orders for review, and obtain records if needed from outside providers.
• Assist in communication with more complex patients, triage via phone.
• Communicate with specialist for patients plan of care.
• Monitor patients charts, assist in scheduling and/or follow up on routine visits-DM/HF/HEP-C.
• For new patients or re-established care-enter all medications on outside providers lists.
• Complete all CGM and Cologuard order forms for provider to review and sign as requested by provider.
• Assist with all other provider request and rooming if needed.
• Preceptor for new staff as needed.
• Ensures provider and care team schedules run efficiently, helping where needed.
• Assists in problem solving with providers, referrals, service issues.
• Monitor and evaluate effectiveness of the care management plan and modify as necessary.
• Follow through with patients’ needs and specific care plan to promote compliance and positive outcomes.
• Associate level medical/nursing or equivalent.
• Completion of an accredited RN program. Current and valid Oklahoma RN license.
• 3+ years of clinical experience preferred.
• BLS CPR Certification.
• Critical thinking skills, prioritization, ability to adapt and take constructive criticism, and frequent changes is tasks with ability to complete in a timely manner.
• Medical terminology, communicate medical results to patients in a way they can understand, and have a good understanding of triage medicine and pathophysiology.
• Basic computer skills and able to navigate multiple medical records systems.
• Experience with RPMS or willingness to learn.
