RN - Case Manager w/ Utilization Review Experience - $89.10/hr in Bozeman, MT

Job Summary (List Format) Registered Nurse Non-Certified Case Manager • Collaborates with physicians and multidisciplinary team to create patient care plans. • Monitors patient progress towards desired outcomes through assessments and record review. • Identifies and addresses barriers to effective patient care. • Actively participates in discharge planning. • Completes comprehensive assessments within regulated timelines to determine case management eligibility. • Develops, implements, and monitors individualized case management plans in collaboration with patients, caregivers, and healthcare team. • Maintains an ongoing caseload, providing regular outreach and management. • Promotes integration of services, including behavioral health and long-term/community care, to ensure care continuity. • Performs ongoing evaluation of care plans, documents interventions and outcomes, and recommends changes as needed. • Utilizes clinical judgment and assessment triggers to identify members' needs. • Ensures timely completion of required education and training per policy and regulations. • Must be a Montana licensed Registered Nurse with at least 2 years of nursing and utilization review experience. • Required certifications: American Heart Association BLS. • Preferred: Bachelor's in Nursing and 2 years acute care hospital case management experience. • Demonstrates strong judgment, communication, organizational, and teamwork skills. • Must be able to work in a busy, stressful environment, maintain confidentiality, and utilize EMR and standard office software. • Requires regular attendance, possible overtime, weekend, and on-call work. • Physical demands include lifting, standing, walking, bending, reaching, and fine motor skills. • Must be able to handle exposures to pathogens, radiation, chemicals, and emotionally challenging situations. • Day shift (7:30am-4:30pm), with weekend rotations.
