The RN Clinical Expediter Patient Logistics play a pivotal role in optimizing patient flow across the healthcare system, coordinating care delivery, and ensuring efficient transitions in our hospitals. Drives efficiency in daily hospital operations and utilizes data and analytics to assist in decision-making. This position requires strong leadership skills, clinical expertise, and the ability to collaborate effectively with various healthcare professionals. Responsibilities The RN Clinical Expediter Patient Logistics responsibilities include, but are not limited to: Patient Flow Coordination: • Perform chart reviews and collaborate with Physicians and Specialists for consultations, admissions, transfers, and discharges. • Evaluate and negotiate appropriate patient placement based on current census, staffing, and acuity considerations. • Expedite, prioritize, synchronize, and mediate real-time clinical issues related to admissions, transfers, and discharges. • Manage deviations from patient care pathways, resolve bottlenecks, and ensure adherence to service level agreements. Leadership and Collaboration: • Serve as a leader and resource to facilitate efficient patient flow activities throughout the hospital. • Collaborate with interdisciplinary teams and other facilities to identify and eliminate barriers impeding patient flow. • Work with care team leadership including, but not limited to, nursing, physical medicine, imaging, physicians, and care management to address patient flow issues and identify process improvement strategies. • Participate in Quality Improvement projects and provide ongoing assistance to interdisciplinary patient care teams. Decision-Making and Communication: • Anticipate and determine strategies for long length of stay or complex discharge patients in collaboration with Case Management. • Analyze guidelines, policies, and protocols to ensure appropriateness and medical necessity of hospitalization. • Communicate effectively with ancillary department leaders to escalate testing and documentation necessary for discharge. • Demonstrate high-level decision-making regarding discharge prioritization in collaboration with the patient logistics team. Education and Training: • Educate staff on admission, transfer, and discharge processes. • Provide training and orientation to less experienced staff. • Act as a resource person in patient flow processes and incorporate evidence-based practice into daily activities. Data Management and Analysis: • Collect, analyze, and report data to identify barriers in achieving quality improvement goals. • Utilize flow tools/systems and electronic medical records to support, manage, and evaluate patient flow throughout the facility. Coordination of Care: • Collaborate with care team to promote the well-being of patients and support patient/family preferences. • Remove barriers to support a holistic care environment. • Seek knowledge and competency about current and new practices, incorporating evidence-based practice into daily activities. Additional Duties: • Cooperatively accept additional duties as assigned. • Participate in interdisciplinary team rounds/huddles and discharge planning to ensure timely and efficient patient movement. Reports to the Director Patient Logistics Hub. This position may have additional or varied physical demand. Exposed to traffic conditions while traveling to various programs or meetings and exposed to hazardous weather conditions when traveling. Exposed to a variety of constant interruptions and some extreme time deadlines. Requirement Talking Yes Seeing Yes Hearing Yes Color Acuity Yes Qualifications • Current RN license, AD required, BSN preferred • Bachelor’s degree in nursing or other related field to healthcare/leadership required • MHA or MBA preferred • Minimum 10 years of clinical experience in a hospital setting. • Strong leadership and communication skills. • Ability to work collaboratively in a fast-paced healthcare environment. • Knowledge of patient flow processes and healthcare system dynamics. • Must be able to communicate in English (Verbal/Written) Physical Demands Strength No Standing/Walking Yes Keyboarding Yes
